A Clinical Study of Breast Disease


Abstract
A clinical review was carried out in 815 cases of breast disease for three years from July 1980 to June 1983 at the department of general surgery of Chonnam National University, Xwang-ju, Korea.
The results were obtained as follows:
1) The most common breast disease was mammary dysplasia (38.5%), followed by fibroadenoma (26.6%), breast carcinoma (14.8%).
2) The prevalent age group of mammary dysplasia was between 35 and 44 years, fibroade noma 25 and 34, and breast carcinoma 45 and 54.
3) The most common chief complaint was palpable breast mass (89.1%), followed by abnormal nipple dischare (4.5%) and breast pain (4,3%).
4) In physical examination, palpable breast mass was found in 95.6% of all patients, and its size was usually 2~5 cm in diameter (41.1%).
5) The most frequent site of lesion was in the upper outer quadrant of breast (39,9%), followed by inner upper quadrat (21.0%) and lower outer quadrant (17.4%). The lesion of left breast was slightly more frequent than that of right breast, but in breast carcinoma, right breast was slightly more frequent.
6) In breast carcinoma, the sensitivity of mammography was 82.0%, specificity 72.7%, accuracy 77.0%, correct positive 71.9%, false negative 17.9% and false positive was 28.1%.
And that results of fine needle aspiration cytology were 84.2%, 54.6%, 73.3%, 76.2%, 15.8% and 23.8% respectively.
7) The overall accuracy of physical diagnosis according to histopathologic examination was 69.7%, and 35.1% of physical malignancy was confirmed as benign and 10.0% of physical benign was confirmed as malignancy
8) According to the TNM system, the most frequent stage of breast carcinoma was stage ¥²(37.2%), followed by stage 11(27.3%), stage ¥°¥±(18.2%) and stage ¥³(7.3%) in that order of frequency.
9) Modified radical mastectomy was performed in 55 cases (66.3%) of breast cancers and standard radical mastectomy in 19 cases (22.9%), simple mastectomy in 7 cases(8.4%), and lumpectomy in 2 cases (2.4%). Most of them ctically.
